The Anthem Premier 1500 plan is the most comprehensive and second lowest deductible Virginia health insurance coverage available from Anthem Blue Cross Blue Shield. Premier plan benefits include:
Deductible – $1,500 per person, maximum of two per family.
Co-insurance – You pay 20% after deductible
Maximum Out of Pocket Expense (MOOP) –$3,500 including the deductible per person, maximum of two per family.
Office Visits – Co-payment of $30 for primary care physicians and $40 for specialists. Unlimited visits.
Lifetime Maximum – Unlimited
Preventative Care – No charge when using in-network doctors for recommended care
Prescription Drugs – Brand name and generics, unlimited benefit. Requires $15 co-pay or 40% co-insurance, whichever is greater.
Professional & Diagnostic Services – You pay 20% after deductible
Inpatient Services – 20% after deductible
Outpatient Services – 20% after deductible.
Emergency Room Services – 20% after deductible
Maternity – Not covered. Maternity benefits are available when choosing the Premier plan with a $2,500 or higher deductible. Requires optional rider for extra $71.00 per month and a six-month waiting period.
