$5,000 benefit - $35.00 per month or $385.00 if paid annually
$7,500 benefit - $41.00 per month or $451.00 if paid annually
$10,000 benefit - $47.00 per month or $517.00 if paid annually

Individual coverage pricing is as follows:

$5,000 benefit - $22.00 per month or $242.00 if paid annually
$7,500 benefit - $28.00 per month or $308.00 if paid annually
$10,000 benefit - $34.00 per month or $374.00 if paid annually

For an additional $11.00 per month, you can also add the "Consult a Doctor" option for free telephone consultations with a licensed physician 24 hours a day, 365 days a year.  This benefit can be used with the basic policy for a fee of $35 per consultation.
